Do you typically avoid lactose, dairy, FD&C dyes, or PEGs (polyethylene glycols)?
While these ingredients may not do much to the average person, sensitive patients know that accidentally consuming them can cause pain, derail an entire day, or even lead to multiple days stuck in bed or a trip to the ER. And while being on a first-name basis with ER staff might sound entertaining, most of us would rather avoid those situations altogether.
Here is the surprising part: did you know that these ingredients are found in nearly every over-the-counter and prescription medication sold in the U.S.?
Many pharmacists and doctors will say the tiny amount used in each pill, capsule, or tablet is too small to affect anyone. But for a select group of patients, those tiny amounts are exactly what spark hypersensitivity reactions.
This is not an issue for everyone, but for those it does affect, the consequences are far from negligible. They can be life-altering.
And here is where it gets validating: recent research is proving that patients who react to excipients are not as rare as once believed. In July 2025, a medical journal published evidence showing that so-called “inert” ingredients in our medications may not be as harmless as assumed.
Ingredients such as polyethylene glycols (PEGs), parabens, and food-coloring dyes, though critical for drug aesthetics, formulation, and stability, have been linked to hypersensitivity reactions. These range from mild redness around the mouth or skin, to throat swelling, to full-blown anaphylaxis, triggered by either immune (IgE and T-cell mediated) or non-immune pathways.
The study also noted that underreporting and uneven safety monitoring across regions make it difficult to know just how widespread the problem really is. Still, it pointed to evolving regulatory changes and even desensitization strategies designed to protect patients.
The reality is that we often focus only on the active ingredients in medications. But excipients, such as fillers, dyes, emulsifiers, and stabilizers, are in every prescription and over-the-counter drug. For patients with mast cell disorders, genetic predispositions, histamine intolerance, IgE mediated allergies, or even religious or ethical concerns, these ingredients can pose very real risks.
What Can You Do With This Information?
Here are three practical ways to approach your pharmacist or healthcare provider if you are hypersensitive or have concerns about excipients
- Say it clearly
“I have a known hypersensitivity to certain excipients such as lactose, dyes, or PEG. Does this formulation contain any of them and is there an alternative manufacturer or formulation without them?”
“Is it possible to transfer my prescription to a local compounding pharmacy to make my medication(s) without the fillers I’m sensitive or allergic to?” - Explain your reason for health, religious, or ethical concerns
Preface that you understand your sensitivity is not common but it is serious for your health; or share that for more or religious reasons, you do not consume ingredients of X kind,
“I follow a strict vegan lifestyle and avoid ingredients such as lactose or gelatin. Can you check if any formulations you carry are free from these?”
I am extremely allergic to dairy products and the lactose in this medication can not with absolute certainty be free of casein or other milk compounds. Can we please help me find a version that’s safe for me to consume?” - Bring specifics and ask for options
“Due to X (name the condition, religion, or allergy), I react to PEGs or FD&C dyes. Could you review the ingredients in the pharmacy’s stock and suggest an alternative without that excipient, please?”
Giving your pharmacist clear context, whether it is allergy, intolerance, or lifestyle, opens the door to exploring alternatives or even contacting other pharmacies or suppliers. Oftentimes, a compounding pharmacy can assist with building the prescription medication sans irritants.
In summary – excipients are not always harmless. Staying informed and communicating clearly with your healthcare team helps you advocate for safer, more suitable medications.
